You need to test the trumps and also to take the Heart finesse to stand any chance of making this contract. From this outrageous bidding you expect East to be very distributional and to hold both Minors from his 3NT bid which can only be “UNT”.
You win the first trick and play one trump to the Queen discovering the 4:0 trump break, not wholly unexpected.
You now take the Heart finesse relieved to see your Queen win. Now if East has three trumps this means you will not have the tempo to unblock the Hearts before drawing the remaining trumps. Therefore after winning the Heart queen you plonk down the Ace hoping East started with Doubleton King. Now it’s a matter of unblocking the Jack of Hearts before drawing all the remaining trumps ending up in dummy. Now you can cash the established hearts and claim your contract by way of 5 Spades, 5 Hearts and 2 clubs, discarding your losing Diamond on the 5th round of Hearts.
