On this hand declarer can rely on Diamonds breaking 33 (28% chance) or he can hope that West holds 4 or more Diamonds along with his marked Queen of Hearts (from the lead). With this in mind declarer runs off ALL his Black Suit winners and now poor West cannot guard both red suits and declarer gets home despite the Diamond suit breaking badly.
